Offline tool for an initial once-daily insulin glargine PFS dose and next-day adjustment using weight, last 06:00 FBS, and renal function. Optional: HbA1c, previous Total Daily Insulin (TDD). Excludes shock/hypothermia.

1) Mandatory inputs

If µmol/L: app converts to mg/dL (divide by 88.4).

2) Optional inputs

If provided (>0), app uses it only to estimate total glargine dose. No subtraction/balance is performed.
Perioperative fasting (pre-op + post-op total) expected > 12 hours
No shock & no hypothermia (required)

Clinical caveats (minimum):
• For DKA/HHS, shock, vasopressors, severe sepsis, hypothermia, pregnancy → follow critical care / IV insulin protocols.
• If type 1 diabetes or pancreatogenic diabetes: basal insulin is mandatory; avoid under-dosing; monitor closely.
• CKD (eGFR < 60) increases hypoglycemia risk → conservative dose and closer monitoring.
• Steroids, infection, enteral/TPN feeds can increase insulin requirement substantially.
• Hypoglycemia treatment in this order uses D25 100 mL IV stat;ensure local availability.
